Policy Statement 5: Telecommunications Services Request

Procedure Description

Requests for telecommunications system design, cost estimates, or related information must be made by submitting a Telecommunications Services Request (Appendix).

The Telecommunications Services Request must include the following information:

1. Name of department
2. Budget to be billed
3. Person authorizing the work to be done
4. Person to be contacted
5. Location for service by building name and room number
6. Detailed description of work requested:
   1. Furnish list of all extensions/equipment affected
   2. If several changes are involved, attach a diagram of area showing location of existing and requested telecommunications services
   3. Date services needed (Allow three weeks)
   4. A list of special instructions, if any
