Faculty and Staff Diving Physicals
ECU faculty and staff require a diving physical to become or remain active divers with the University. The details of the physical examination are listed in the ECU Diving and Water Safety Manual.
To setup a diving physical examination:

Telephone the Division of Perspective Health, at 252-744-2073 or 252-744-2070, to make an appointment. Perspective Health can explain how to get to Dr. Swinker's office.

Sign the Applicant's Release Of Medical Information Form located on the last page of the Medical Evaluation Of Fitness For Scuba Diving Report.

Fill out the Diving Medical History Form.

Carry the forms with you to the examination.
Note: Prospective Health has a list of faculty/staff requiring diving physicals for this calendar year. If you have questions about when your medical expires, contact the Diving & Water Safety Office.
