The I-9 is a U.S. Citizenship and Immigration Services form. It is used by an employer to verify an employee's identity and to establish that the worker is eligible to accept employment in the United States.
The NC-4 form is filled out so that an employer can withhold the correct amount of state income tax from your pay.
The W-4 form is an Employee's Withholding Allowance Certificate IRS form that determines how much Federal Withholding Taxes will be deducted from an employee's paycheck.

Substance Abuse Policy
This document details ECU's policy on substance abuse, including information on available counseling, rehabilitation, and disciplinary actions to prevent substance abuse.
This form indicates your agreement with ECU's Confidentiality Agreement, indicating that all confidential ECU information that your position at ECU accesses is done so in the performance of your employment.
Wage and Hour Policy
This form indicates your agreement with the ECU Wage & Hour Compensatory Time-Off Agreement, indicating that you understand that the State gives "time off" compensation in overtime rather than monetary compensation whenever possible.
Selective Service Compliance
This form indicates your compliance with the requirements of the Military Selective Service Act, indicating you are registered with the Selective Service or not registered because of a specific reason.
This form is used to verify qualifications by verifying transcripts from institutions, licensing boards, etc.
Receipt of Information Form
This form will need to be printed and signed by you indicating that you have received a copy of the SPA Employee Handbook, and agree to abide by the policies/procedures set forth in the Application Policy for New Employees, the University Student & Employee Computer Use Policyy, and the ECU Patent & Copyright Agreement.
