The application packet will consist of the following pieces of information:
Application materials should be sent via email to Gloria F. Snead (firstname.lastname@example.org), preferably scanned into one PDF document. Applications and all other materials must be received by midnight October 30, 2014 for consideration for the 2015 Leadership Academy. Application decisions will be made by December 9th. Sessions for the 2015 Chancellor’s Leadership Academy will being on January 10th, and conclude April 10th.

- Resume’/Leadership Profile (maximum two pages for resume)
- Completed Supervisor Statement form
- A letter of support from your direct supervisor
- Two additional letters of recommendation
