Download MS Word template of DRV-1 Form
Using the MS Word template download, you can save the file to your computer, complete the form using the template, print it, and send it to: ECU Central Motor Pool. You must have Microsoft Word.

Download Adobe Acrobat file of DRV-1 Form
If you download the Adobe Acrobat PDF file, and you do not have the correct software to complete the form on the screen, you will need to print the PDF file, hand write or type your responses, and send it to Central Motor Pool.

Driver verifications for state-owned vehicles will be based on a point system, rather than the number of driving convictions. The driver policy has been updated and may be reviewed on-line in the ECU Business Manual:
