As your medical home we are responsible for arranging, coordinating and enhancing your access to care through the use of health information technology, electronic medical records, open scheduling, expanded hours and a variety of ways to communicate with you.

On this page of our website you will find support to manage your chronic medical conditions.

Our team of doctors, nurses, nutritionists, pharmacists, dentists , psychologists have designed patient education materials for you. This does not replace our discussions with you about your heart failure or diabetes, or high blood pressure (hypertension) but gives you some tools to help you understand your condition and manage it to the best of your ability.

Your doctor will have established a plan of care with you. The materials on this page will help you carry out that plan. If you have questions or suggestions , please do not hesitate to ask.
