Insurance Company Requirements for Gastric Bypass Surgery
Below you will find a chart of insurance requirements. This chart is not meant to be used as the final word on insurance requirements. This is only meant to be used as a guideline. Please check with your individual insurance company as to their specific requirements, policies and whether or not weight loss surgery is a covered benefit under your plan or if it is excluded. We will be submitting for insurance approval once you have completed your testing. As you can see some insurance companies require a great deal of testing prior to having the surgery.
