The Department of Health Education & Promotion has forms and applications for each of its programs. Several of these forms are for your benefit (ie: to help you keep track of your classes or to offer class scheduling suggestions). Other forms and applications are required for a particular Health Education & Promotion program. Please be aware that some forms and applications are due by a specific date.
