Forms

No event can be done without paperwork. Participant medical/registration and liability waiver forms must be filled out for participation in any of the mentioned events.  A volunteer service learning form and a volunteer liability waiver form must be filled out for volunteering in any of the mentioned events.  A scholarship form, limit up to $100, is also available to begin your own active living.
