Order Cell Phones, Pagers, Calling Cards and Broadband Cards

Departments are required to order cell/pager service, calling cards and Internet broadband service cards through the ITCS Network Services office.

Step 1. Download the order form

From the Telecommunications Devices Requests section on the left of the screen, choose the appropriate form (see the graphic below). Instructions for completion are included.

Step 2. Choose a plan

The Discount Schedule for authorized vendors is below. All eligible discounts are applied at the time of the order.

AT&T Wireless	Service Plans - 20%	Equipment - 50%	Accessories - 25%
US Cellular	Service Plans - 15%	Equipment - 50%	Accessories - 10%
Verizon Wireless	Service Plans - 22%	Equipment - 25%	Accessories - 25%

Step 3. Complete and forward the form

Complete the form, including signatures and account numbers. Fax, e-mail or send interoffice to the appropriate network services office for your campus.
