EDUCATIONAL PROGRAM FORM

If your chapter is scheduling an education presentation/program for new members and/or chapter members, please complete the following form and return if to the Office of Greek Life.  Please be sure to complete the form for each educational presentation/program in order to document the educational programming that occurs within your chapter.

Documentation is due by the last day of each month to the Greek Life Office for the program to count toward the Standards of Excellence.
