RHA Forms

This page is where you can submit your Of The Month (OTM) forms. Completing an OTM is a formal way of bringing attention to a program that was done on campus by an RA, a Hall Government or by RHA. OTM forms are submitted, on a monthly basis, into a on campus competition. A winning OTM may be selected to be entered into the state, regional, and national competition as well.

Please use the General and/or Program OTM Online Form below to submit your OTM information. Use the General and Program OTM links above to navigate to the two forms. OTM forms are due the 5th of every following month; when submitting an entry for March that OTM would be due by the 5th of April.
