We take all comments very seriously and believe it is one of the best ways we have to know how others feel about the care we provide at Student Health Services (SHS). There are a couple of ways to leave a comment for us:
You can choose to keep your comment anonymous or, if you provide us with your name, e-mail and/or mailing address, we will follow up on the comment with a formal response.
While we welcome comments from everyone, please be aware that if you are submitting a comment about someone else's visit (e.g. a parent commenting on their child's visit), we will be unable to discuss any details of that visit without a written release from the patient giving us permission. To obtain a medical release, please visit our forms page.

Please remember that this comment form is not confidential nor anonymous. Please do do not communicate any personal medical or confidential information in your comments. SHS will not divulge any medical information in their response back to you.
