Medication Form (required if medication is to be administered)

Please return the required forms to 128 Student Recreation Center, Greenville, NC 27858 prior to the start of camp or on the first day of camp. Your child will not be able to participate unless we have current forms for them on file. Please call 252-328-6387 with questions.
