Accommodation Letters

During the first week of every term enrolled, it is the students' responsibility to pick up copies of their accommodation letter.  This letter is to be hand-delivered by the student to their instructors as it notifies the instructor of the individuals' approved accommodations.  Students taking on-line courses or students in the Nursing program should e-mail DSS at dssdept@ecu.edu and request an electronic copy of their accommodation letter.  Presentation of this letter to faculty is required.

 
Alternative Test Request Form

Students who wish to take their exams at the DSS Testing Center must fill out and submit the Alternative Text Request form.  Completed forms must be received by DSS at least 48 business hours prior to the exam date.  Unless approved by a staff member from DSS, late forms will not be accepted.

Electronic Textbook Request Form

To request electronic versions of your textbooks please download and fill out the Electronic Textbook Request Form.  Be sure to include a copy of your book list.  You can download your booklist from the ECU OneStop site.

 
Interpreting Request Form

Students requesting an interpreter for academic courses and university sponsored events must complete and submit the Interpreting Request Form two business days prior to the event.
