If you have questions regarding your immunization history, immunization requirements, or questions in general please call Student Health Service at (252) 328-6841. You can also e-mail general questions to our immunization department at immunizations@ecu.edu. Please remember that emailed questions and responses have no guarantee of confidentiality. Please do not e-mail questions containing specific confidential information with regards to your health.  Student Health will not reveal confidential information concerning your health in the e-mail reply. 

 

ECU Immunization and Medical Record Requirements


 

Students taking four or less credit hours on campus and/or students enrolled in online courses only, Do Not have to meet the immunization requirements for University enrollment. Exemption is automatically given at time of course registration.

 

Students taking five or more credit hours on campus must meet the North Carolina State Law Immunization requirements below:

 

 

If Immunization requirements are not met, dismissal from school is mandatory under North Carolina Law.  If you have not completed your immunizations you have a minimal amount of time to complete all requirements.  Immunizations can be obtained at local clinic, medical offices and at ECU Student Health Service.

 

 

To meet East Carolina Immunization and Medical record requirements, that were mailed to you with your ECU enrollment package, please do the following: (If you do not have a copy of the form you may download a copy   to mail in).    

                                           

Immunization Record

1.          Please fill out the student information on the top of the Immunization Record.

2.          Have a physician or clinic complete each required Immunization section that is required for the student and sign the form using black ink. (See What is Required )  Records from other medical offices clinics are acceptable and may be attached to the form. (See Where to Get Your Records

  Report of Medical History

3.          Please complete the "Report of Medical History" in its entirety and sign at the bottom of the form

4.          Mail all forms together to

          May 15th for students entering in the Fall Semester
   December 15th for students entering in the Spring Semester

Students accepted after these deadlines must submit all immunization records no later than the first day of classes. All immunizations should be complete at that time.

              

Or you may download a copy of the forms to mail in.  East Carolina University does NOT require the physical exam to be completed but other Universities do.  You do not need to have a physical exam in order to attend ECU.

 

6.       Incoming students who do not fulfill all their immunization requirements will be notified by mail of their status and immunization deficiencies.
