Vessel Reservation Form (Print/fill out, return to DWS Office)  Float Plan (Print/fill out, give to someone in your department or family)  Boating Activities Under Auspices of ECU Waiver Form (Print/fill out, return to DWS Office)  Statement of Agreement to Abide Form (Print/fill out, return to DWS Office)  Accident Report Form (If there is an accident print/fill out, return to DWS Office)  Check-Out Form For Vessels Less Than 16 Feet Long.  ORV Check-Out Form For Vessels 16 to Less Than 26 Feet Long.  ORV Check-Out Form For Vessels 26 to Less Than 40 Feet Long.  Check-Out Form For Vessels 40 to Less Than 65 Feet Long.  Check-In Form for all Vessels (must be filled out by all operators when you return your vessel, submit the form to the DWS Office).  Sea Service Form (must submit the form to the DWS Office).
