Frequently Asked Immunization Questions


Who needs to complete the Immunization Form/Report of Medical History? All incoming students enrolled to the University for the fall and/or spring semester and will be attending classes on the ECU campus. (Students who are attending classes off-campus, auditing classes, taking Internet classes, or satellite classes DO NOT have to complete these forms. Exemption is automatically given at time of course registration.

What Immunizations do I need? The requirements vary upon age. Please see theWhat is Required section for a listing.

What Immunizations does an International Student need? All the required immunizations are based upon their age. Please see the What is required section. Additionally, International Students are required to have a TB skin test with a negative result within the 12 months preceding the first day of classes.

Where do I get the Immunization Form/Report of Medical History? Forms are mailed out to each newly accepted student with their enrollment package. If you did not receive a form or have misplaced it you may download a form or call ECU Student Health Service (252) 328-6793 and we can mail another copy to you.

When are the forms due?

May 15th for students entering in the Fall Semester

December 15th for students entering in the Spring Semester

Students accepted after these deadlines must submit all immunization records no later than the first day of classes. All immunizations should be complete at that time.

Where do I get my immunization records from for submittal to ECU Student Health Service to meet the Immunization requirements? Please see Where to Get Your Records

Can I get my Immunization shots from ECU Student Health Service when I get to School? Yes, please refer to Where/When to get Immunizations at The ECU Student Health Service for more information regarding times that clinics are available.

Will I be notified if the information I submit is incomplete? Yes, a Student Health Service representative will contact you by mail, of missing information.

If I have specific questions regarding immunizations who can I contact? Persons with questions regarding their immunization history, immunization requirements, or questions in general should call Student Health Service at (252) 328-6841. You can also e-mail general questions to our immunization department at immunizations@ecu.edu. Please remember that emailed questions and responses have no guarantee of confidentiality. Please do not e-mail questions containing specific confidential information with regards to your health. Student Health will not reveal confidential information concerning your health in the e-mail reply.
