Chronic Disease Self-Management Programs Leaders

The Office of Healthy Aging Research, Education, and Services (OHARES) aims to advance population health by increasing access to and utilization of Stanford University's evidence-based Chronic Disease Self-Management Programs [i.e., the Chronic Disease Self-Management Program (CDSMP), Chronic Pain Self-Management Program (CPSMP), and Diabetes Self-Management Program (DSMP)].  Studies evaluating the effects of CDSMP participation among adults with a myriad of chronic conditions (e.g., cardiovascular disease, diabetes, and chronic obstructive pulmonary disease) across the lifespan have documented the following: (1) improvements in participants' health behaviors, health status - - i.e., well-being, quality of life, symptom intrusion, and level of functioning; (2) improvements in self-efficacy - - i.e., individual's belief that he/she has the knowledge and skills to effectively self-manage his/her chronic conditions; and, (3) decreases in participants' emergency department and inpatient hospital utilization.  As a licensed provider of these programs, OHARES partners with various community-based organizations to (1) increase access to and utilization of such programs by residents in rural and often impoverished parts of eastern North Carolina and (2) train Leaders to implement such workshops in these often under-served areas.  Emphasis is placed on the implementation of barrier-free access to CDSMP workshops - - i.e., all workshops are free-of-charge for participants and all leader trainings are free for people who are interested in running these workshops for under-served populations.
