ProxCards and Alarm Codes

To request a new or replacement ProxCard:

  1. Log into SODM Assist.
  2. Under Request Type select "Informatics Support” then choose “ Request Forms” then choose “ ProxCard”,  fill in the needed information.
  3. A photo is needed for a card to be issued.
  4. To attached a photo go to Attachments and click on Add Files. NOTE: Ross Hall photos are taken by Peggy Novotny and CSLCs by the Bushiness Manager.

Ross Hall
When an SODM | Assist request is received, ProxCards are created by Informatics and delivered to Clinical Affairs to be setup for axiUm approvals. When you pick up your card from Clinical Affairs or Informatics it is ready for use. 

CSLCs
When an SODM | Assist request is received, ProxCards are created by Informatics and delivered to Clinical Affairs to be setup for axiUm approvals. 

NOTE: CSLC ProxCards are mail to the clinics non- activate. Once the card arrives the Business Manager will need to send a request to Informatics to activate each card.

NOTE: If you will be traveling from Ross Hall to the CSLCs and require door access, please note that in the ticket. 


To request an alarm code for the CSLCs:

  1. Log into SODM Assist.
    • Under Request Type select "Informatics Support” then choose “ Request Forms” then choose “ Alarm Code”,  fill in the needed information. 
Once approved the alarm code will be emailed to the user from Paul T. Cuthrell (cuthrellp@ecu.edu)

NOTE: Students that are performing clinical rotations are emailed codes at the beginning of the semester. The code is valid at all CSLCs. 

NOTE: Please make sure to check your email and store your code in a safe location. DO NOT share your alarm code with anyone!
