Qualtrics is a Web-based survey software for East Carolina University faculty, staff and students. Before creating a survey, ensure you have met the University IRB criteria.
Please take note that East Carolina University has transitioned the university's existing regulation regarding administration of surveys from interim to permanent status. The regulation may be found in the University Policy Manual under University Communications in the Governance and Administration functional category at http://www.ecu.edu/PRR/01/30/01.
Are You a STUDENT Creating a Survey On Behalf of a Department/Faculty/Staff?
If YES, the survey should be owned by the faculty/staff person. As a student, you must be added as a collaborator on the survey. As a collaborator, you will still have the capability to create/edit the survey.

Qualtrics is a HOSTED CLOUD system.; As a HOSTED CLOUD system, it is recommended that you, as the survey owner, back up your data regularly to Piratedrive.
Qualtrics is NOT APPROVED for the collection or storage of institutional (HR & Financial), FERPA, HIPAA, or sensitive data. DO NOT COLLECT institutional, FERPA, HIPAA, or sensitive data using Qualtrics.
East Carolina University requires that all data collected or stored in Qualtrics meet federal, state or industry requirements. As a research best practice, all sensitive or confidential research data should be de-identified prior to collection or use. Please follow IRB research guidelines and contact the ITCS Help Desk at 252.328.9866/800.340.7081 to request an IT Security consultation if you have questions concerning appropriate protocols for the collection and storage of sensitive or confidential data.
Qualtrics provides services and uses software and technology that may be subject to U.S. export controls administered by the U.S. Department of Commerce, the U.S. Department of Treasury Office of Foreign Assets Control, and other U.S. agencies and the export control regulations of Switzerland and the European Union.
